"How" indicates a manner in which they may be similar; "Why" simply asks for a reason for them being similar. You're not asking for the reason or reasons that men are like high heels, are you? I think that the point of your joke was to indicate that you feel that men are similar to high heels, and the punchline to your joke was intended to explain the basis for the comparison. But that's just my $0.02 :)

What's the main reason women wear high heels?
When I see a woman in heels, I always thought it was for sensual reasons. Recently I read an article when a (very conservative) woman said she wore high heels as a confidence booster. Question for women: Why do you wear your high heels? Question for men: What do you think of a woman when you see her in high heels?

There are so many, many reasons why I wear heels. They make me feel feminine and desirable. It thrills me that men start to turn heads even when they hear me coming, and when they see the height of my heels, sometimes their eyes seem to be popping out of their heads, lol. Heels shape my hips and bum and make my legs look a lot longer. I can also need the extra height, I'm only 5'3. Heels make me feel both confident and vulnerable. I feel like a queen, but at the same time I know I cannot run very fast. They actually feel nice on my feet. I read somewhere that feet are very sensitive, and that's totally true. The higher the heel, the more sensual the feeling is. I am getting aware of every step I take. That happens especially in ballet heels which are my new favourite :-) Actually I can hardly think of any reason for a woman NOT to wear heels ;-)
